Vancouver Coastal Health (VCH) is seeking a dedicated Registered Nurse to contribute to the Primary Care Network. Relocation assistance may be available, and nurses may qualify for provincial and federal student loan forgiveness.
As a Registered Nurse within the Primary Care Network, you will play a crucial role in delivering comprehensive care to patients in collaboration with a multidisciplinary team.
Key Responsibilities:
Conduct assessments, formulate care plans, implement interventions, and evaluate outcomes for patients and their families. Collaborate with Primary Care Providers to ensure adherence to the local care delivery model, co-designed by various health partners. Utilize principles of health promotion, chronic disease management, and self-care to assess patient needs, identify challenges, and provide direct care. Work closely with patients and the interdisciplinary team to ensure continuity of care, facilitating transitions and coordinating services. Serve as a nursing resource for the team, applying critical thinking and problem-solving skills to enhance patient care.About Primary Care Networks:
Primary Care Networks (PCN) are integral to the provincial Primary Health-Care Strategy, fostering local partnerships among health authorities, family practice divisions, and First Nations communities. These networks consist of diverse teams, including nurses, pharmacists, and allied health professionals, who work alongside family physicians and nurse practitioners to provide holistic care.
The collaborative approach within PCNs aims to improve patient access to primary care services, support physicians in their practice, and enhance the range of skills available to patients. This model ensures that patients receive comprehensive care tailored to their needs.
Qualifications:
Education & Experience
Current registration as a Registered Nurse with the BC College of Nurses and Midwives (BCCNM). Minimum of one (1) year of recent clinical experience relevant to the designated area, or an equivalent combination of education and experience. Valid driver's license and access to a vehicle.Skills & Competencies:
Proven ability to apply critical thinking skills within safe patient care limits. Understanding of primary care principles and patient-centered care models. Knowledge of professional regulations and standards related to community health nursing. Competence in evidence-based clinical practices relevant to primary care. Familiarity with provincial regulations and guidelines related to community and primary care. Ability to provide culturally safe care to diverse populations. Strong communication skills and the ability to work independently while prioritizing patient care. Proficiency in using computerized patient care documentation systems.
