Position Overview

Vancouver Coastal Health (VCH) is seeking a Registered Nurse to contribute to the Primary Care Network in Powell River, BC. Relocation assistance may be provided, and nurses could qualify for Provincial and Federal student loan forgiveness.

In your role as a Primary Care Nurse with VCH, you will:

Evaluate, plan, implement, and assess nursing care for patients and their caregivers. Collaborate with the Primary Care Provider in line with the local PCN care delivery model, co-designed by PCN Partners. Utilize health promotion, chronic disease management, advance care planning, and self-care management principles to perform assessments, identify issues, deliver direct care, and address any discrepancies from the care plan. Work alongside the patient and interdisciplinary team to ensure continuity of care by coordinating ongoing services and facilitating transitions. Serve as a nursing resource for the team by applying critical thinking and problem-solving skills, providing insights based on nursing theory and practice.

ABOUT PRIMARY CARE NETWORKS:

Primary Care Networks (PCN) form the foundation of the provincial Primary Health-Care Strategy. This innovative care model is managed locally through partnerships between Health Authorities, Divisions of Family Practice, and First Nations communities. PCNs consist of multidisciplinary teams of nurses, pharmacists, and allied health professionals, collaborating with family physicians and nurse practitioners in various healthcare settings. PCNs aim to provide robust connections between patients and primary care providers, hospitals, and diagnostic services.

Team members involved in patient care will work collaboratively to gain a comprehensive understanding of healthcare needs. This new structure enables patients to access a full spectrum of healthcare options more efficiently, ensuring they can establish a relationship with a family physician or nurse practitioner and engage with a diverse health team.

The team-based care approach within Primary Care Networks seeks to enhance patient access to primary care services, support existing physicians in maintaining or expanding their practices, and broaden the range of skills and expertise available to patients during their care journey. It ensures that the necessary systems and tools are in place to support patients in achieving better health outcomes.

Qualifications

Education & Experience

Current practicing registration as a Registered Nurse with the BC College of Nurses and Midwives (BCCNM). A minimum of one (1) year of recent, relevant clinical experience or an equivalent combination of education and training. A valid driver's license and access to a vehicle.

Knowledge, Skills & Abilities

Proven ability to apply critical thinking skills within safe patient care limits. Comprehensive understanding of primary care principles and patient-centered care models. Familiarity with relevant Professional Regulations and Standards, including Community Health Nurse of Canada (CHNC) competencies. Knowledge of evidence-based clinical practices related to primary care. Understanding of provincial acts, regulations, and policies concerning home and community care. Proficiency in managing acute and chronic diseases, mental health, and self-management support. Ability to provide culturally safe care across diverse populations. Competence in promoting Indigenous Cultural Safety. Understanding of Trauma-Informed Practice and Harm Reduction principles. Capability to identify when referrals to other healthcare professionals are necessary. Skills to facilitate seamless transitions in care. Effective communication and conflict resolution abilities. Capacity to work independently and prioritize care effectively. Knowledge of other health disciplines and their roles in patient care. Ability to utilize research in practice. Understanding of adult education principles to teach and coach patients and caregivers. Proficient computer skills, including experience with patient care documentation systems. Competence in CPR techniques. Physical capability to perform job duties. Ability to operate relevant equipment.
