Currently, Bissell Centre is looking for an individual wanting to join the **Home Visitation** team. Are you ready to be our **Family Support Navigator** and make an impact in our community?

**ABOUT THE ROLE**

**Duties and Responsibilities**

**Program Duties**
- Administer and support families to access the emergency Diapers/Formula program at the Bissell Centre.
- Administer Health for Two program as required. Manage distribution and storage of Health for Two items such as Vitamin D, grocery coupons, and bus tickets.
- Manage bus passes provided by PATH program. Ensure timely and accurate entry of bus pass information into PATH spreadsheet.
- Provide referrals and support to access other community support as required to meet daily basic needs.
- Build rapport and maintain professional relationships with participants.
- Complete Starting Points assessment 1 with participants to determine their own/family strengths, resources, needs and aspirations to provide greater opportunities for enhanced support to participants.
- Complete Starting Points Assessment 2 for participants referred to case management.
- Assess and work with participants based on their own readiness to make positive changes and increase self-reliance.
- Provide crisis intervention support as required.
- Make appropriate referrals to health and community services and, when necessary, advocate for families to receive those services.
- Monitor intakes for case management, triage, and complete warm transfers to Family Support Workers for longer term support.
- Conduct participant surveys and submit within deadlines.
- Develop and facilitate group programming with the intention to strengthen daily living skills and reduce social isolation and loneliness.

**General and Administrative**
- Maintain paper files, ensure accurate and timely entry of case notes into ETO, accurate CIR and WCB incident reports within established time standards.
- Maintain a current and accurate file for each participant.
- Ensure ETO transfers are completed for participants moving from basic needs to case management.
- Utilize ETO, ADP, Teams, and Outlook.
- Provide latest program information and updated statistics as required.
- Maintain and submit accurate mileage and expense logs for reimbursement.
- Participate in probationary and annual performance reviews.
- Participate in ongoing learning and training opportunities as identified in annual performance review.
- Attend bi-weekly scheduled staff and supervision meetings.
- Participate in Community Safety Walks on rotating schedule as required.
- Perform other duties as required.

**Qualifications**

**Education and Experience**
- Degree or diploma in Social Science or related field and/or comparable experience with children and families.
- Employees with Social Work Diploma/Degree/Master’s are required to provide and maintain ongoing registration, in good standing, with Alberta College of Social Workers (ACSW) as per _Alberta Health Professions Act._
- Minimum 1-year related employment experience. E.g., families and children, children’s services, vulnerable, marginalized, or at-risk populations.
- Alberta Class 5 License, satisfactory driver’s abstract and vehicle.

**Knowledge, Skills, and Abilities**
- Commitment to a strength-based and client centered approach.
- Knowledge of the roles, functions, and activities of case management.
- Strong, written, and oral communication skills.
- Conflict Resolution and problem solving.
- Solution focused, good time management, strong organizational, and advocacy skills.
- Effective assessment skill and ability to collaboratively develop goal plans with caregivers and complete evaluation.
- Knowledge about community resources, health services, pre/post-natal supports.
- Knowledge about the Child and Youth Enhancement Act.
- Ability to maintain a flexible work schedule.
- Knowledge of Indigenous cultures, history, current issues, and marginalized communities who have experienced trauma.
- Direct experience with issues of poverty, homelessness, and Edmonton’s urban core are required.
