POSITION TITLE: Independent Living Support Coordinator

PROGRAM: Operations – Independent Living Support

Off-site at Alberta Health Services (AHS)

POSITION TYPE: Temporary (1 year contract), Full-time

HOURS OF WORK: 37.5 hours per week

POSITION SUMMARY: This position will report to Off-site Team lead for the program. The Coordinator is responsible for delivering services in accordance with the CMHA Calgary Region service model. Through coaching and mentoring, you will support clients with a mental illness and/or concurrent diagnosis on their journey to wellness by connecting them to community resources and developing their daily independent living skills.

REPORTS TO: Team Lead, Independent Living Support

KEY SERVICE RESPONSIBILITES AND SPECIFIC ACCOUNTABILITIES

  • Adheres to the CMHA Program Policies and Procedures within the assigned Program.
  • Advocates for clients when necessary and to ensure they receive adequate care, treatment services, and information from the community and CMHA.
  • Assists clients in establishing peer, family, and community support networks.
  • Assist clients with developing practical skills that will prepare them for independent community living.
  • Collaborate with a client’s clinical support on a regular basis to ensure they are made aware of a client’s needs, progress, and any other identified issues that fall beyond the scope of both programs.
  • Develop practical skill workshops or modules relevant to the clients of the program as needed.
  • Facilitate workshops and skills groups to clients and Alberta Health Services (AHS) staff.
  • Liaises with community partners on a regular basis to ensure collaborative efforts in assisting clients to meet their personal goals.
  • Participate in community liaison meetings as required.
  • Record, update and submit client demographics and service information to assist with the generation of required agency statistical records and funding reports.
  • Maintains effective written and verbal communication systems necessary for client service delivery and program functioning.
  • Provides clients with the coaching, support, and connection to resources necessary for the maintenance or development of living skills in the areas of home management, mental health management, money management, interpersonal relationships, leisure/recreational pursuits, self-care and coping skills, medication management, problem solving, and psychosocial skill development and maintenance.
  • Other duties as required.

ADDITIONAL TEAM RESPONSIBILITIES AND COMPETENCIES

  • Adhere to the ‘Working alone policy” as outlined in the Outreach Services Policies and Procedures manual.
  • Adhere to the definition and guiding principles of the ILS Service Models.
  • Mentoring new staff, practicum students and volunteers as required.
  • Participate as a responsible member of the agency.
  • Participate in the overall functioning and coordination of the two programs.
  • Be a resource to the community in the area of information and referral services.
  • Able to engage in active listening and provide/accept constructive feedback.
  • Able to represent self and the agency in a professional manner.

QUALIFICATIONS AND SKILLS

  • Minimum Education – Bachelor of Social Work (BSW) preferred or recognized diploma or undergraduate degree in a human services area.
  • Two (2) years of experience in working with persons with psychiatric disabilities, diagnosed mental illness and/or addictions along with experience in life skills.
  • You have worked in the mental health field for over two (2) years and have a deep commitment to and understanding of the recovery model. You demonstrate deep respect not only for clients but for the teams you support, your peers and community partners.
  • Applied Suicide Intervention Skills (ASIST) Training.
  • First Aid and CPR certifications.
  • Valid driver’s license and a vehicle.
  • Experience working in an interdisciplinary setting.
  • Flexible and adaptable.
