OVERVIEW
This position provides money management, case management and support services to mental health clients within the Canadian Mental Health Association Durham (CMHA Durham), Collaborative Living.
MAJOR RESPONSIBILITIES
Case Management Services
- Participates as a member of a high performance, multi-service team managing an assigned caseload of mental health clients within our collaborative living and community settings.
- Conducts assessments and prepares individualized supportive client care plans that promote long-term stability and growth.
- Develop, implement, evaluate and revise treatment goal plans in collaboration with clients.
- Ensures that clients understand CMHA Durham services and agree to the expectations related to Collaborative Living using appropriate service agreements.
- Works with clients in one-on-one and/or group situations teaching life skills that promote sharing of responsibilities associated with reaching their goals to move onto independent housing.
- Provides services in a respectful individualized, supportive and flexible manner.
- Supports and assists clients with maintaining their skills to live independently in the community and promotes safe and healthy living standards ensuring that client’s basic needs of food, clothing and shelter are met.
- Provides support services to clients assisting with scheduled appointments, arranging transportation to appointments and/or accompanying clients to appointments, as necessary.
- Supports clients to establish and foster peer supports and natural family connections.
- Establishes supportive therapeutic relationship with clients and advocates for, and with, clients.
- Consults and collaborates with community resources and service providers ensuring clients obtain the most appropriate services and receive continuity in their care.
- Provides on-call services to clients and acts as back up coverage to staff, as required.
- Documents client contacts, client participation/involvement, assessments, and other reports in a timely manner, as outlined in the CMHA Durham policies and procedures.
- Performs other tasks as assigned by management.
Trustee Services
- Provides support for individuals to develop the necessary tools to manage their financial affairs at the highest degree of independence possible.
- Provides quality client service, developing trusting rapport and respectful communication.
- Manages, track and ensure that client’s funds are disbursed in such a way that their basic needs are met and that their expenses are paid in a timely manner.
- Evaluates current skill level of clients to provide the necessary skill training to enable clients to develop and implement short term and long-term plans for meeting their financial well being.
- Collaborates with other programs, ensuring the sharing of tools and resources, and provide Trustee support where necessary.
- Offers education money management advice to assist individuals to arrive at the highest degree of independence possible.
- Develops and promote partnerships with clients, family members and other service providers as appropriate.
- Negotiates with creditors/collection agencies and advocates for client rights.
- Supports clients with preparation of yearly income tax returns.
RISK MANAGEMENT
- Minimizes risks adhering to CMHA Policies and Procedures identifying, assessing, evaluating and controlling risks ensuring well-being of staff and clients.
SKILLS AND QUALIFICATIONS
- Post-secondary education in a related area of study (i.e., mental health, social sciences, or human services) and a minimum of two years related experience or equivalent combination of education and experience.
- Sound knowledge of the Mental Health Act, Health Care Consent Act, social service benefits and related legislation in Ontario.
- Strong Communication skills, including listening skills, and motivational interviewing with the ability to advocate for clients and collaborate with multiple stakeholders.
- Ability to work in a wide variety of environments such as offices, on the road, viewing apartments, and client dwellings.
- Ability to work with diverse cultures.
- Successful candidates will bring the following attributes to the position: initiative, confidence, compassion, flexibility, creative decision-making, solid judgement skills, persuasiveness, community knowledge, and strong boundary making skills.
- Knowledge of budgeting and financial management required.
- Ability to acquire and utilize community resources effectively to meet clients’ needs.
- Knowledge of the Ontario Disability Support Programs (ODSP), Ontario Works (OW), Canadian Pension Plan (CPP), Old Age Security (OAS), social service benefits and related legislation in Ontario.
- Valid Ontario Driver’s license, travel is required.
- Knowledge of MS Office (Word, Excel, PowerPoint, and Outlook).
- Intermediate proficiency in written and oral French is an asset.
EFFORT
- Sits at a desk, as well as stands and walks daily, in order to perform the duties of the job.
- Concentration, attentiveness, and creativity required daily to attend meetings, develop the necessary tools to help clients manage their financial affairs, manage a caseload, prepare documents, listen to clients, etc.
WORKING CONDITIONS
- Works on site in office setting, as well as off-site in residential homes, doctors’ offices, shelters, communal living environments, staff vehicle, and clinical office environments.
- Occasional exposure to abusive/irate clients, distractions, interruptions, and deadlines with little exposure to hazards.
