About the Role

We are seeking a highly skilled Behavioural Support Personal Support Worker to join our team at Ina Grafton Gage Home. As a Behavioural Support Personal Support Worker, you will play a vital role in providing compassionate care to our residents, supporting them in managing responsive behaviours and promoting a high quality of life.

Key Responsibilities
  • Provide person-centered care to residents, supporting them in managing responsive behaviours and promoting a high quality of life.
  • Collaborate with the Director of Care and Behavioural Support Manager/Lead to develop and implement behaviour care plans.
  • Support residents and families in managing responsive behaviours, and provide specialized care for individuals with dementia and mental health disorders.
  • Participate in behaviour rounds with internal and external resources to contribute to the development of person-centered care planning.
  • Assist with the implementation of behaviour care plans and external resource recommendations.
  • Provide 1 to 1 support for newly admitted residents with new or emerging responsive behaviours.
  • Participate in continuous quality improvement of the RHA and associated care services.
Requirements
  • Current Personal Support Worker Certificate from a recognized institute.
  • Experience working with residents with behaviours/formal training (GPA/PIECES) an asset.
  • Completion of six-hundred and forty (640) hours in PSW curriculum.
  • Minimum 1-year experience in a Long-Term Care setting an asset.
  • Knowledge of Ministry of Long-Term Care- Fixing Long Term Care Act and Regulations.
  • Ability to work well with others in a team approach and adapt to changing situations.
  • Ability to work effectively in a multidisciplinary stressful environment.
  • Excellent written and verbal communication skills, interpersonal skills and computer literacy.
  • Ability to work with diverse clients.
  • Valid First Aid & CPR Certification.
  • Must be available to work weekends and statutory holidays.
