About the Role

We are seeking a highly skilled and compassionate Behavioural Support Personal Support Worker to join our team at Ina Grafton Gage Home. As a Behavioural Support Personal Support Worker, you will play a vital role in providing person-centered care to our residents, supporting them in managing responsive behaviors and promoting a high quality of life.

Key Responsibilities
  • Provide Person-Centered Care: Assume responsibility for the provision of person-centered care on the resident home area, ensuring the physical, psychological, social, and spiritual needs of each resident are met.
  • Support Residents and Families: Communicate with residents and families using person-centered language, engaging them in the provision of their own care to support meaningful connections and a quality lived experience.
  • Participate in Multidisciplinary Care: Participate in all aspects of personal resident care, working collaboratively with other team members to ensure the delivery of optimal care and services.
  • Implement Behaviour Care Plans: Implement behaviour care plans, and external resource recommendations as directed, mentoring and coaching staff in the care plan activity handover process.
  • Provide 1 to 1 Support: Provide 1 to 1 support for newly admitted residents with new or emerging responsive behaviors, and support reintegration of residents after psychiatric leave or long stay on an external behavioral unit.
  • Collaborate on Behaviour Rounds: Participate and collaborate on behaviour rounds with internal and external resources to contribute to the development of person-centered care planning.
  • Contribute to Continuous Quality Improvement: Participate in continuous quality improvement of the RHA and associated care services, contributing to the development of person-centered interventions on the care plan to minimize responsive behaviors and promote daily living skills and tasks that spark joy to residents and their family members.
Requirements
  • Current Personal Support Worker Certificate: Possess a current Personal Support Worker Certificate from a recognized institute.
  • Experience in Long-Term Care: Minimum 1-year experience in a Long-Term Care setting, with experience working with residents with behaviors/formal training (GPA/PIECES) an asset.
  • Knowledge of Ministry of Long-Term Care Regulations: Knowledge of Ministry of Long-Term Care- Fixing Long Term Care Act and Regulations.
  • Excellent Communication and Interpersonal Skills: Possess excellent written and verbal communication skills, interpersonal skills, and computer literacy.
  • Ability to Work in a Multidisciplinary Environment: Ability to work well with others in a team approach and adapt to changing situations.
  • Valid First Aid & CPR Certification: Possess valid First Aid & CPR Certification.
  • Availability to Work Weekends and Statutory Holidays: Must be available to work weekends and statutory holidays.
