Job Summary

WoodGreen is seeking a highly skilled and compassionate Personal Support Worker to join our Supportive Housing team. As a Personal Support Worker, you will play a vital role in providing high-quality care and support to seniors who have been street involved, experiencing mental health and/or addictions issues.

Key Responsibilities
  • Practice a client-centered and culturally competent approach to care.
  • Assist clients with activities of daily living, including feeding, transfers, bathing, skin care, oral hygiene, toileting, and continence care.
  • Assist clients with instrumental activities of daily living, including light housekeeping duties, laundry, meal preparation, and bulk shopping.
  • Perform non-controlled acts assigned by the Supervisor, which have been taught by a Regulated Health Professional (or Supervisor), and in accordance with the Unit's policy.
  • Perform controlled acts which have been delegated by a Regulated Health Professional, and in accordance with the Unit's policy.
  • Provide medication assistance or reminders in accordance with the Unit's Medication Assistance Policy.
  • Continuously observe clients and their environments, and report and document unsafe conditions and behavioural, physical, mental and/or cognitive changes to the Supervisor.
  • Complete and maintain progress notes in client profile binder and the communication log.
  • Communicate and collaborate with caregivers/stakeholders to determine preferred approaches to care.
  • Respond professionally to responsive behaviours; if unable to deal with behaviours successfully, report to Supervisor and work with various professional and resource persons as required to support the client.
  • Apply proper food handling practices in accordance with the Unit's policy.
  • Ensure there are sufficient program supplies, forms and equipment available; report to Supervisor when supplies are low.
  • Ensure the anti-racism objectives and community development principles of the organization are achieved within the program.
  • This position will include some evening and weekend shifts monthly and is subject to exposure to second-hand smoke, odors, pests and clients' potential use of inappropriate language or swearing.
Requirements
  • Personal Support Worker Certificate.
  • NACC Certificate.
  • 2+ years' experience working with older adults with mental health, addictions issues and street involvement.
