Clinical Social Worker Needed to Provide Comprehensive Social Work and Therapeutic Clinical Services to Patients and Families in a Rapidly Growing Organization. Job Description: We are seeking a skilled Clinical Social Worker to provide comprehensive social work and therapeutic clinical services to patients and families. The successful candidate will conduct comprehensive social work and psychosocial assessments, provide therapeutic clinical services, and consult with patients, families, and healthcare professionals. Responsibilities: • Conduct comprehensive social work and psychosocial assessments of patients and families • Provide therapeutic clinical services, including grief and bereavement counseling • Consult with patients, families, and healthcare professionals on high-risk factors, recovery, and current practice in the field of adaptation • Maintain patient records and document patient assessments and charting information • Provide leadership to the interdisciplinary team in patient care planning • Communicate with community health and social agencies to determine alternate levels of care and effective utilization of patient resources • Participate in local committees and planning bodies to identify areas for improved efficiency and effectiveness in service delivery • Develop, coordinate, and deliver in-services, presentations, psycho-educational and support groups • Initiate, develop, and carry out research activities Qualifications: • Master's degree in Social Work from an approved school of Social Work • One year of recent, related acute care clinical experience • Current full registration with the BC College of Social Workers • Comprehensive knowledge and understanding of emotional and mental health issues related to illness, injury, and patient and family cultural dynamics • Ability to educate and provide consultation to patients, families, and healthcare professionals • Ability to develop, coordinate, and deliver in-services, presentations, psycho-educational and support groups • Ability to provide leadership to the interdisciplinary team in care planning for the effective utilization of patient resources About the Organization: Fraser Health is a leading healthcare organization serving over two million people in Metro Vancouver and the Fraser Valley.
