Summary
Social Worker required to assess patients for multi-organ transplant readiness, develop intervention plans, and provide clinical teaching to students and colleagues.

Job Description
The University Health Network, a leading research teaching hospital, is seeking a skilled Social Worker to join its team. As a key member of the inter-professional team, you will be responsible for assessing patients for multi-organ transplant readiness, developing and implementing intervention plans, and providing clinical teaching to students and colleagues.

**Key Responsibilities:**

• Assess patients for multi-organ transplant readiness, considering social needs and supports for a successful transplant
• Develop and implement intervention plans to facilitate changes in behavior, attitudes, feelings, and/or the environment
• Provide clinical teaching to Social Work students, Medical students, and professional colleagues
• Participate in relevant research activities and projects
• Collaborate with community agencies to provide services and support to patients
• Complete applications for Alternate Level of Care (ALC) facilities and establish contact with members of the inter-professional team and community agencies

**Qualifications:**

• Master's Degree in Social Work
• Registration with the Ontario College of Social Workers and Social Service Workers
• Membership in the Ontario Association of Social Workers (preferred)
• Three years of experience working with an inter-professional team in a hospital setting (preferred)
• Excellent verbal and written communication skills
• Excellent interpersonal, time management, and organizational skills
• Research experience/education (an asset)
• Student education/supervision (an asset)
• Knowledge of policies, legislation, programs, and issues related to social welfare institutions and services
• Knowledge of a second language (an asset)
