Pharmacist - Outpatient Specialist

We are seeking a highly skilled and experienced Pharmacist to join our team at the University Health Network. As an Outpatient Pharmacy Specialist, you will play a critical role in providing exceptional patient care and ensuring the safe and effective use of medications.
Key Responsibilities- Medication Management: Review, verify, and dispense prescribed medication orders, ensuring their appropriateness and accuracy.
- Drug Therapy Monitoring: Monitor drug therapy for effectiveness, toxicity, and adherence to evidence-based medicine guidelines.
- Medication Reconciliation: Conduct medication reconciliation to ensure accurate and safe medication use.
- Incident Response: Respond to medication incidents and promote safe medication practices on the unit.
- Drug Information: Provide accurate and timely drug information to healthcare professionals.
- Leadership and Supervision: Provide direct and/or functional supervision of technician staff, ensuring their development and growth.
- Education: Completion of a university degree in pharmacy (BSc or PharmD) or equivalent.
- Certifications: Active registration with the Ontario College of Pharmacists (Part A) and a certificate of competency from the Pharmacy Examination Board of Canada.
- Experience: Proven experience working in an outpatient pharmacy setting, with a strong understanding of medication management and dispensing processes.
- Skills: Excellent communication, interpersonal, and customer service skills, with the ability to work effectively in a team environment.
