Position Overview

Department: Allied Health
Reports to: Manager, Allied Health
Hours: Full-time
Salary: Competitive and commensurate with experience
Shifts: 5 shifts per week
Status: Permanent Full-Time

The successful candidate will be responsible for:

  • Evaluating patients' readiness for multi-organ transplant from a psychosocial perspective.
  • Assessing patients to determine service needs and developing treatment recommendations.
  • Collaborating with patients, families, and the inter-professional team to implement intervention strategies aimed at fostering positive behavioral and emotional changes.
  • Continuously reassessing and adjusting intervention plans as necessary.
  • Coordinating with community agencies to enhance patient support.
  • Completing necessary applications for Alternate Level of Care (ALC) facilities and facilitating communication among the inter-professional team and community resources.
  • Delivering immediate services that benefit both the hospital and the community.
  • Providing educational support to Social Work students and medical professionals.
  • Engaging in relevant research initiatives and projects.
  • Contributing to the operational efficiency of the Social Work department.

Required Qualifications

  • A Master's Degree in Social Work.
  • Registration with the Ontario College of Social Workers and Social Service Workers.
  • Preferred membership in the Ontario Association of Social Workers.
  • A minimum of three years of experience working within an inter-professional team in a healthcare setting.
  • Experience in acute care is preferred.
  • Strong verbal and written communication skills.
  • Excellent interpersonal, organizational, and time management abilities.
  • Proven problem-solving skills.
  • Research experience is an asset.
  • Experience in student education and supervision is advantageous.
  • Familiarity with policies and legislation related to social welfare services.
  • Knowledge of a second language is beneficial.
