Key Responsibilities- Medication Management: Review, verify, and dispense prescribed medication orders, ensuring their appropriateness and accuracy.
- Drug Therapy Monitoring: Monitor drug therapy for effectiveness, toxicity, and adherence to evidence-based medicine guidelines.
- Medication Reconciliation: Conduct medication reconciliation to ensure accurate and safe medication use.
- Incident Response: Respond to medication incidents and promote safe medication practices on the unit.

- Education: Completion of a university degree in pharmacy (BSc or PharmD) or equivalent.

- Experience: Proven experience working in an outpatient pharmacy setting, with a strong understanding of medication management and dispensing processes.
