About the Role- Provide direct patient care by assessing, planning, providing, and evaluating nursing care to patients in the Renal Services department.
- Collaborate with patients, families, caregivers, physicians, and members of the inter-professional care team to develop, implement, and modify care plans.
- Conduct comprehensive nursing assessments, identify risk factors, and develop nursing interventions to improve patient outcomes.
- Participate in care and discharge planning, ensuring effective implementation and ongoing modification of care plans.
- Coordinate nursing care, prioritize tasks, and delegate responsibilities to support staff as needed.
- Collaborate with the inter-professional care team to identify and resolve issues, changes in patient condition, and adjustments to care plans.
- Teach patients and families/caregivers on illness prevention, health restoration, and health maintenance to support self-care and discharge planning.
- Document assessments, nursing diagnoses, observations, interventions, patient/family responses, outcomes/evaluations, and referrals according to practice standards and Fraser Health procedures.
- Conduct comprehensive nursing assessments of patients on admission and at regular intervals.
- Collaborate with patients and their families/caregivers, physicians, and members of the inter-professional care team in the development, implementation, and ongoing modification of care plans.
- Participate in care and discharge planning, ensuring effective implementation and ongoing modification of care plans.
- Coordinate nursing care, prioritize tasks, and delegate responsibilities to support staff as needed.
- Collaborate with the inter-professional care team to identify and resolve issues, changes in patient condition, and adjustments to care plans.
- Teach patients and families/caregivers on illness prevention, health restoration, and health maintenance to support self-care and discharge planning.
- Document assessments, nursing diagnoses, observations, interventions, patient/family responses, outcomes/evaluations, and referrals according to practice standards and Fraser Health procedures.
- Graduation from an approved School of Nursing.
- Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- Two (2) years' recent, related clinical nursing experience in the designated clinical area and completion of post-graduate coursework and/or certification applicable to the designated clinical area, or an equivalent combination of education, training, and experience.
