About the Opportunity

We are seeking a skilled Health Care Social Worker to join our interdisciplinary healthcare team. As a key member of our team, you will provide collaborative, holistic care on the primary to quaternary care continuum.

The successful candidate will be primarily concerned with the relationships between people and their environments, which are integral to psychosocial, emotional and spiritual wellbeing. They will build upon the strengths of patients/clients by focusing on existing coping strategies and problem solving skills, past successes, and personal resources.

About You

We are looking for a candidate with a Bachelor's degree in Social Work from a recognized university and current registration with the Nova Scotia College of Social Workers. The ideal candidate will have 2 years' post BSW related clinical experience and previous experience working in a community-based setting.

The successful candidate will possess excellent communication skills, the ability to complete biopsychosocial assessments, and the ability to assist patients and families with navigation of local community resources and agencies. They will also have the ability to demonstrate effective time management and disciplined self-organization, as well as comfort and skills working collaboratively with community partners.

Key Responsibilities
  • Provide collaborative, holistic care on the primary to quaternary care continuum
  • Build upon the strengths of patients/clients by focusing on existing coping strategies and problem solving skills, past successes, and personal resources
  • Assist patients and families with navigation of local community resources and agencies
  • Complete biopsychosocial assessments
  • Work collaboratively with community partners
Requirements
  • Bachelor's degree in Social Work from a recognized university
  • Current registration with the Nova Scotia College of Social Workers
  • 2 years' post BSW related clinical experience
  • Previous experience working in a community-based setting
  • Excellent communication skills
  • Ability to complete biopsychosocial assessments
  • Ability to assist patients and families with navigation of local community resources and agencies
  • Valid Nova Scotia Driver's license and access to a reliable vehicle
