Health Care Social Worker

Nova Scotia Health is seeking a skilled Health Care Social Worker to join our team. As a member of our interdisciplinary health care team, you will provide collaborative, holistic care on the primary to quaternary care continuum.

About the Role

  • Provide social work services to patients and families, focusing on psychosocial, emotional, and spiritual wellbeing.
  • Build upon the strengths of patients/clients by focusing on existing coping strategies and problem-solving skills.
  • Develop and implement individualized care plans, taking into account the social determinants of health.
  • Collaborate with other healthcare professionals to provide comprehensive care.

About You

  • Bachelor's degree in Social Work from a recognized university.
  • Current registration with the Nova Scotia College of Social Workers.
  • 2 years' post BSW related clinical experience required.
  • Previous experience working in community-based settings preferred.
  • Demonstrated ability to use a client-/family-centered approach.
  • Familiarity with various evidence-based treatment approaches.
  • Excellent communication and interpersonal skills.

Working with Us
