The Pediatrics on Princess (POP) Centre is seeking a highly motivated and compassionate CSP Advocate to join our Clinical Team. As a key member of our team, you will work closely with children and their families to provide support and guidance in overcoming social-emotional, behavioural, and academic challenges.

Job Summary:

The CSP Advocate will be responsible for providing advocacy services to children and families, ensuring they have access to necessary resources and support to achieve their goals. This is a 3-month contract position, with a possibility of renewal.

Key Responsibilities:
  • Conduct referrals, screenings, intakes, assessments, and follow-up processes with children and their families.
  • Develop and implement treatment plans with clear goals, objectives, timelines, and discharge plans.
  • Identify and mitigate barriers to attendance and treatment plan goals.
  • Perform regular check-ins to support families in achieving their goals.
  • Support research and evaluation activities in collaboration with the Horizon Health Network research team.
  • Support skill development and parenting strategies with caregivers.
  • Maintain accurate and up-to-date patient records in the Electronic Medical Records (EMR) and Outcome Tracker (OT) databases.
  • Conduct outreach activities with schools, government, community agencies, providers, and others.
  • Support and assist with group programs.
  • Promote the best interests of children in accordance with the United Nation's Convention of the Rights of the Child.
Qualifications:
  • A bachelor's degree in social work (BSW) is recommended. An equivalent combination of education and experience may be considered.
  • Experience working with children and families will be considered a strong asset.
  • Ability to attend work on a regular basis in-person, with occasional work during the evenings and weekends.
  • Written and spoken competency in English is required.
  • Criminal record and vulnerable sector checks are required.
  • Proficient in Microsoft 365, Office, Outlook, and ability to learn new systems.

What We Offer:

We offer a competitive hourly rate of $26 to $30, depending on education and experience. Our team is dedicated to providing a supportive and inclusive work environment.
