About the Opportunity

We are seeking a skilled Community Outreach Worker to join our team at Nova Scotia Health Authority. As a key member of our healthcare team, you will provide a range of clinical services to support individuals and families in our community.

Key Responsibilities:

  • Provide screening, assessment, and brief intervention services to individuals and families
  • Develop and implement individualized treatment plans to support recovery and wellness
  • Collaborate with community agencies and organizations to provide navigation and advocacy services
  • Provide education and support to clients and families on mental health and addiction issues
  • Participate in interprofessional teams to develop and implement care plans

Requirements:

  • Bachelor of Social Work degree or equivalent
  • Registration with a professional regulatory body
  • One year post-BSW clinical experience in mental health and addiction
  • Screening and risk assessment skills, including suicide risk assessment
  • Ability to work independently and as part of an interprofessional team
  • Excellent communication and interpersonal skills
