We are seeking a skilled Community Outreach Worker to join our team at Nova Scotia Health Authority. As a key member of our healthcare team, you will provide a range of clinical services to support individuals and families in our community.
Key Responsibilities:
- Provide screening, assessment, and brief intervention services to individuals and families
- Develop and implement individualized treatment plans to support recovery and wellness
- Collaborate with community agencies and organizations to provide navigation and advocacy services
- Provide education and support to clients and families on mental health and addiction issues
- Participate in interprofessional teams to develop and implement care plans
Requirements:
- Bachelor of Social Work degree or equivalent
- Registration with a professional regulatory body
- One year post-BSW clinical experience in mental health and addiction
- Screening and risk assessment skills, including suicide risk assessment
- Ability to work independently and as part of an interprofessional team
- Excellent communication and interpersonal skills
