Community Outreach Worker - Community Wellness

Mount Carmel Clinic is a leading provider of community-based services, committed to promoting health and well-being in underserved populations. We are seeking a highly motivated and compassionate Community Outreach Worker to join our team.

About the Role

  • Function as part of a coordinated service delivery team, working proactively with new and existing clients to provide support and recovery services.
  • Build relationships with clients, providing services in a non-judgmental and culturally appropriate manner, and helping them to achieve their goals.
  • Work collaboratively with other team members to ensure seamless service delivery and achieve desired outcomes.

Key Qualifications

  • Valid drivers' license and access to a reliable vehicle.
  • Minimum of 2 years' experience working in community, preferably with the program's target population.
  • Experience providing outreach services to hard-to-reach populations.

Essential Skills and Abilities

  • Proven knowledge and ability in relationship-building, critical thinking, facilitation, decision-making, problem-solving, and conflict-resolution skills.
  • Demonstrated ability to prioritize and focus work-related activities to effectively and efficiently achieve service goals and produce expected results/outcomes.
  • Commitment to the principles that guide Mount Carmel Clinic, including diversity, equity, cultural safety, and social determinants of health.
