About the Role

VON Canada is seeking a skilled Nurse Care Specialist to join our team. As a key member of our community health team, you will provide high-quality patient care in the comfort of clients' own homes.

Key Responsibilities
  • Provide comprehensive patient assessments and develop personalized care plans
  • Deliver evidence-based care to clients with complex health needs
  • Collaborate with interdisciplinary teams to ensure seamless care coordination
  • Communicate effectively with clients, families, and healthcare providers
  • Participate in quality improvement initiatives to enhance patient outcomes
Requirements
  • Current registration as a Registered Nurse in the province of practice
  • Canadian Nurses Association certification in Community Health Nursing an asset
  • Experience in community health or home care an asset
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
