VON Canada is seeking a skilled Registered Nurse to join our team as a Community Health Specialist. In this role, you will provide high-quality, client-centered care to individuals and families in their homes and communities.
Key Responsibilities- Client Care and Management: Develop and implement individualized care plans that meet the unique needs of each client, ensuring timely and effective delivery of services.
- Nursing Assessment and Intervention: Conduct comprehensive assessments of clients' physical and emotional needs, and provide evidence-based interventions to promote optimal health outcomes.
- Education and Health Promotion: Provide education and support to clients and their families to promote independence, self-care, and healthy lifestyle choices.
- Collaboration and Communication: Work collaboratively with other healthcare professionals, community agencies, and clients' families to ensure seamless care coordination and effective communication.
- Quality Improvement and Risk Management: Participate in quality improvement initiatives and contribute to the development of policies and procedures to ensure safe and effective care delivery.
- Education and Certification: Current registration as a Registered Nurse in the province of practice, with a minimum of 2 years of experience in community health nursing.
- Skills and Abilities: Excellent communication and interpersonal skills, with the ability to work independently and as part of a team. Strong assessment and problem-solving skills, with the ability to prioritize tasks and manage multiple responsibilities.
- Professional Development: Commitment to ongoing learning and professional development, with a focus on staying current with best practices in community health nursing.
This role requires a high level of physical activity, including lifting, carrying, bending, and walking. The ability to work in a fast-paced environment and adapt to changing priorities is essential.
