Seeking a Social Worker 1 to coordinate and provide quality care to patients and families in an interdisciplinary team setting, requiring a Bachelor's degree in social work and strong interpersonal skills.

Job Description:

As a member of the interdisciplinary EMP team, the Social Worker is responsible for coordinating and providing quality care within the scope of practice to patients and families.

Key Responsibilities:

  • Coordinate and provide quality care to patients and families within the scope of practice.
  • Develop and implement care plans in collaboration with the interdisciplinary team.
  • Provide education and support to patients and families on health-related topics.
  • Collaborate with community services to ensure comprehensive care.
  • Maintain accurate and confidential records.
  • Participate in quality improvement initiatives.

Requirements:

  • Bachelor's degree in social work from an approved school of social work.
  • Member in good standing with the New Brunswick Association of Social Workers.
  • Knowledge of the target population of Extra-Mural Services and available community services.
  • Valid New Brunswick Class 5 Driver's License and good driving record.
  • Valid and clean criminal record check.
  • Current CPR Level C certification.
  • Strong interpersonal skills, good judgment, and decision-making skills.
  • Ability to work efficiently as a member of the team and possess leadership and coaching skills.
  • High regard for confidentiality.
  • Excellent work habits and professional appearance.
