We are seeking a highly skilled and experienced Pharmacist to join our team at the University Health Network. As a key member of our Outpatient Pharmacy team, you will play a critical role in ensuring the safe and effective dispensing of medications to our patients.
Key Responsibilities- Review and verify medication orders to ensure appropriateness and accuracy
- Identify and resolve drug-related problems to prevent adverse reactions
- Monitor medication therapy to ensure effectiveness and adherence to evidence-based guidelines
- Conduct medication reconciliation to ensure accurate and up-to-date medication lists
- Provide direct and/or functional supervision of technician staff to ensure high-quality patient care
- Respond to drug information inquiries from healthcare professionals and patients
- Perform other duties consistent with the job classification, as required
- Completion of a university degree in Pharmacy (BSc or PharmD) or equivalent required
- Certificate of competency from the Pharmacy Examination Board of Canada required
- Active registration with the Ontario College of Pharmacists (Part A) required
- Certificate of competency from an accredited smoking cessation program and flu vaccination injection program required
- Familiarity with common drug regimens and dispensing processes for specialty medications
- Experience working as part of a multi-disciplinary team to provide high-quality patient care
- Strong interpersonal, communication, and customer service skills
- Working knowledge of adherence programs, including compliance packaging, refill synchronization, and refill management
- Manual billing, out-of-province billing, and specialty medication billing experience
- Experience handling pharmacy inventory and managing stock levels
- Excellent verbal and written communication skills
- Excellent customer service and interpersonal skills
- Excellent organizational and time management skills
- Hospital outpatient pharmacy practice experience preferred
- Hospital pharmacy residency an asset
- Experience handling blister packs an asset
