Resident Care Coordinator

We are seeking a highly skilled and experienced Resident Care Coordinator to join our team at Fraser Health Authority. As a key member of our multidisciplinary team, you will provide clinical supervision, guidance, and support to nursing and nursing support staff, and coordinate clinical team members within the assigned unit(s).
Key Responsibilities- Develop and implement individual care plans to meet the ongoing needs of residents/clients, in collaboration with other clinical team members and residents'/clients' families.
- Supervise nursing and nursing support staff, observe work, and correct where necessary, assign and adjust work assignments, schedules staff, monitors and recommends staff hours of work, including overtime.
- Coordinate the work of the clinical team overall on a day-to-day basis, monitor the quality of care, and the safety of residents/clients, and take corrective action as appropriate.
- Support quality resident/client care and excellence in practice by promoting interprofessional collaboration and team building, supporting staff to plan and carry out resident/client/family education regarding chronic illness, palliative care, etc.
- Participate in or lead the investigation of complaints by residents/clients/families, follow up on complaints, and implement corrective action as appropriate.
- Initiate/lead/support plan of care conferences and rounds for the purpose of optimal resident/client care management according to best practice.
- Identify learning requirements of staff, and provide input into the development of learning objectives, and contribute to the achievement of the learning objectives.
- Participate in the orientation of new staff, and provide input into the development of a comprehensive orientation program, in collaboration with the Educator and other clinical staff.
- Current practicing registration as a Registered Nurse and/or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- Three (3) years of recent related clinical experience in the designated clinical area, including one (1) year of supervisory experience, or an equivalent combination of education, training, and experience.
- Certification applicable to the designated clinical area.
- Clinical proficiency/expertise in the designated clinical area.
- Ability to supervise and provide leadership and work direction.
- Ability to coordinate resident/client assignments, in collaboration with staff.
- Ability to monitor staff workload and resident/client factors, and adjust resident/client assignments accordingly.
- Ability to establish priorities for resident/client care and overall daily operations of the designated unit(s).
- Ability to monitor the quality of resident/client care, and ensure maintenance of standards for nursing practice and nursing support staff.
