Community Inclusion Worker
This role is an opportunity to make a meaningful difference in the life of an individual who requires support to participate in community activities. As a Community Inclusion Worker, you will empower and enable the person to achieve their goals and live independently.
Key Responsibilities:
- Respect and advocate for the person's rights and choices
- Build effective relationships with the person and their support network
- Coordinate and facilitate community-based activities that promote social, vocational, and volunteer participation
- Develop and implement individualized service plans that support the person's goals and preferences
- Record and maintain accurate and confidential information about the person's activities and health
- Collaborate with colleagues and professionals to ensure high-quality care and support
Requirements:
- Grade 12 education
- Community Support Worker Diploma or equivalent
- Minimum 2 years of experience in community social services or healthcare
- Current Emergency First Aid/CPR and Food Safe certificates
- Behavior management training
- Excellent communication and interpersonal skills
- Valid BC Driver's License
- Criminal record clearance for children and vulnerable adults
- Moderate computer literacy
- Eligibility to work in Canada
