Position Overview
As a Community Care Facilitator, you will be an integral part of a dedicated team, providing essential support and assistance to individuals residing in community-based homes. Your role will focus on empowering those we serve to achieve independence and make choices that enhance their quality of life, all while upholding the highest standards of care, health, and safety.
Key Responsibilities
- Advocate for the rights and preferences of individuals receiving support
- Engage effectively with clients to foster meaningful interactions
- Encourage full participation of individuals in their community
- Contribute to the creation, execution, and documentation of Personalized Service Plans
- Build and maintain positive relationships with clients' families, professionals, colleagues, and community members
- Assist clients with daily living activities and medication management
- Demonstrate a solid understanding of behavior management techniques, including mental health considerations
- Document daily activities and personal health information in our case management system
- Collaborate with clients to ensure their living environment is clean and well-kept, including meal preparation and light housekeeping tasks
- Completion of Grade 12 education
- Community Support Worker Diploma or equivalent qualification
- A minimum of 2 years of experience in community social services or healthcare
- Current certifications in Emergency First Aid/CPR and Food Safety
- Prior training in behavior management
- Strong verbal and written communication skills with team members, professionals, families, and community members
- Ability to communicate non-verbally
- Possession of a Restricted Class 4 Driver's License (for Full-Time roles)
- Clear Driver's Abstract
- Criminal record clearance for working with children and vulnerable adults
- Recent TB Test
- Moderate proficiency in computer use
- Eligibility to work in Canada
