Job Title: Personal Support Worker

VON Canada is seeking a Personal Support Worker to join our team in Stratford. As a Personal Support Worker, you will provide personal support and homemaking services to clients and families within the framework of VON policies, procedures, and standards.

Key Responsibilities:
  • Observe and evaluate client needs to determine if changes to the care plan are required.
  • Contribute to the development of the personal support care plan under the direction of the Personal Support Program Manager.
  • Implement changes to the care plan as directed by the Manager.
  • Report changes in client needs or home situation to the immediate supervisor or Personal Support Program Manager.
  • Perform personal support tasks, including light cleaning, meal preparation, and household safety monitoring.
  • Provide personal care, including assistance with walking, climbing stairs, and personal hygiene.
  • Support clients' families by providing child care as required.
  • Organize daily activities and prioritize tasks under the direction of the Personal Support Program Manager.
  • Complete daily call checks and other forms as required.
Requirements:
  • Certificate in Personal Support Worker certification or Developmental Services Worker diploma.
  • Minimum of 2 years experience working as a PSW or DSW.
  • Current Standard First Aid Certificate or CPR level C Certificate.
  • Some experience supporting individuals with dementia and responsive behaviors is preferred.
