Job Summary:

The Hospital Based Case Coordinator will receive referrals and conduct assessments to determine eligibility for Home Care program supports. You will work collaboratively with clients, families, and healthcare teams to develop, coordinate, and evaluate plans of care that support discharge from acute care settings. Your expertise will also be utilized to determine and plan for the best care environment for clients, whether it be a personal care home or alternate care setting.

Responsibilities:
  • Conduct assessments to determine eligibility for Home Care program supports
  • Develop, coordinate, and evaluate plans of care to support discharge from acute care settings
  • Collaborate with clients, families, and healthcare teams to ensure seamless transitions
  • Determine and plan for the best care environment for clients
  • Provide professional intervention as needed
  • Liaise with local healthcare services, hospitals, community partners, and families
Requirements:
  • Minimum three years recent related nursing experience required
  • Experience with Hospital-Based Case Coordination an asset
  • Graduate of an approved Registered Nursing education program required
  • BN, BScMH preferred
  • Valid Manitoba Class 5 Driver's license required and use of a vehicle
  • Responsible for maintaining and providing proof of registration with the College of Registered Nurses of Manitoba (CRNM)
Preferred Qualifications:
  • Wound Care & Diabetes courses an asset
  • Canadian Community Health Nurses Certification CCHN or relevant applicable certification an asset
  • Competence in Procura software and assessment tool interRAI-HC preferred
Working Conditions:
  • Available to work days, evenings, and weekends
  • Use of a reliable motor vehicle suitable for all environmental conditions
  • May be exposed to infectious diseases, blood, and body fluids, toxic materials, noise, allergens, physical and emotional stress
  • May encounter aggressive and/or agitated clients/visitors

This position requires a current satisfactory Criminal Records Check (including Vulnerable Sector Search), Child Abuse Registry Check, and Adult Abuse Registry Check as conditions of employment. The successful candidate will be responsible for any service charges incurred.
