Under the direction of the Program Manager, the Lead Transitional Discharge Worker is primarily responsible for ensuring seamless, safe and supportive transition of individuals from mental health and addictions medical care to community home settings.

The Lead Transitional Discharge Worker is responsible for the coordination of transitioning individuals from the care of hospitals and linking them to community-based mental health and addiction and other support services.

The worker will provide overall lead clinical consultation to the team to ensure the effective transfer of individuals, individual's information and recommendations to optimize successful community re-integration.

Hours: 32.5 per week

Wage Range:
$ $37.25 per hour

Must have:

Able to work flexible hours; certified in CPR and Standard First Aid; may require travel to the District, have a valid Ontario Driver's License and have access to a personal vehicle; a criminal records/vulnerable sector check is required upon hiring.
