Key Responsibilities
  • Coordinate and arrange transportation for clients returning home, utilizing local services or agency vehicles as necessary.
  • Assist clients in understanding and executing hospital discharge instructions, while developing personalized care plans based on assessments and client feedback.
  • Organize errands to fulfill clients' physical needs for recovery at home, including nutrition, medication, and medical supplies, in line with the service plan.
  • Manage home-related tasks such as cooking, cleaning, and laundry, as outlined in the service plan prior to discharge.
  • Engage clients and caregivers by providing essential information regarding the transition from hospital to home and strategies to prevent readmissions.
  • Communicate safety recommendations and assist clients and caregivers in addressing any concerns.
  • Offer companionship to older adults experiencing social isolation.
  • Conduct home risk assessments and evaluate clients' fall risks using standardized tools.
  • Support clients post-discharge by facilitating access to resources and follow-up appointments.
  • Guide clients and caregivers through system navigation.
  • Document client observations and needs electronically, adhering to established standards and reporting to the Program Coordinator.
Qualifications
  • Completion of a minimum of two years of post-secondary education in social services, gerontology, therapy assistance, or a related field, along with three years of relevant experience or a suitable combination of education and experience.
  • Familiarity with medical and technical terminology is advantageous; candidates lacking this knowledge are expected to acquire it during their employment.
  • Proficiency in both English and French is mandatory.
  • Strong computer skills, particularly in Microsoft Office.
  • Experience with CIMS and other databases is a plus.
  • A valid driver's license and reliable vehicle access are required.
Work Environment
  • This role involves working Monday to Friday across various settings, including office environments, hospitals, community locations, and client homes.
  • Travel between locations is necessary, including potential overnight stays for longer distances.
  • Regular working hours are 35 hours per week, with the possibility of irregular hours.
  • The position may require physical activities such as bending, stretching, and assisting clients with mobility.
  • Eligibility to work in Canada is essential; candidates must ensure they meet the requirements for employment.
  • Successful candidates will undergo a pre-employment screening process, including a police information check and proof of COVID vaccination.
