  • Job Description
***: Reporting to the Community Paramedicine Supervisor the Community Paramedicine Patient Navigator is responsible for collaborating with allied health care partners to identify community health and social resources, establishing referral pathways, building networks within the community, creating and maintaining a database of community services. The Patient Navigator will support patient navigation for complex clients by linking clients with health, social and community services and programs.

DUTIES AND RESPONSIBILITIES:

  • Conduct consultations with clients/family to obtain relevant information to ensure the client or family are connected sufficiently to meet their needs.
  • Identify client referral opportunities and connection with external support services where possible.
  • Identify service gaps and areas of duplication in the community.
  • Empower clients, Community Paramedic and Paramedics to learn how to navigate and access a wide variety of community resources, social assistance, and other relevant programs and resources, and assist them through these care pathways.
  • Facilitate communication with community resources, particularly supportive care for the complex population and share information to provide the best experience for the client.
  • Consult/collaborate with other members of allied care teams to discuss and develop an appropriate individualized care plan with the client/family/caregiver based on best practice.
  • Provide health education and health promotion information to individuals, families and caregiver.
  • Interact with members of multidisciplinary health care teams, allied emergency care providers, the public and patients in a professional manner.
  • Collaborate with local community services to develop referral pathways.
  • Create and maintain a database of community services.
  • Ensure all notes and patient charts are updated in a timely manner.
  • Participate in regular team meetings, and other meetings as requested.
  • Maintain confidentiality and privacy of information as per The Municipal Freedom of Information Protection and Privacy Act (MFIPPA), The Personal Health Information Protection Act (PHIPA), Personal Information and Electronic Documents Act (PIPEDA) and in accordance with County of Simcoe Policies and Procedures.
  • Work in accordance with Occupational Health and Safety legislation, and the CSPS Standard Operating Procedures.
  • May be required to report to other work sites within County of Simcoe.
  • Related duties as assigned.

EDUCATION, TECHNICAL SKILLS AND CERTIFICATION:

  • Current or previous Healthcare Professional (Primary Care Paramedic, Advanced Care Paramedic, Registered Nurse etc).
  • Be able to attend conferences, seminars, and participate in mandatory training sessions to keep abreast of changes/trends affecting areas of responsibility.
  • Have a demonstrated understanding of adult learning principles and ability to provide education to clients and families with consideration to the level of health literacy or presence of impairments that may be present among clients and families.
  • Have knowledge of and demonstrated ability to incorporate core competencies including customer service, communication, teamwork, flexibility/adaptability, initiative/selfmanagement, accountability, and resiliency.
  • Demonstrated ability to work independently and in a team environment.
  • Strong communication skills and the ability to foster a positive and cooperative working relationship with multidisciplinary health team members, patients, other agencies and the public.
  • Criminal Records Check with Vulnerable Sector Screening that is less than ninety (90) days old at time of hire.
  • Valid Ontario Driver's License and the ability to travel throughout County of Simcoe for work related purposes.

EXPERIENCE:

  • Minimum three (3) years experience working in a community, hospital or primary health care setting, with a broad knowledge of local health and social services.
  • Experience working within and understanding of the Ontario health care system, specific to provincial & local community health services.
  • Experience with community seniors and mental health an asset.
