What you will do:

Reporting to the Manager, Community Programming in the Community Programming and Outreach Services Division, your primary responsibilities will include, but are not limited to, the following:

  • Daily supervision of programs for older adults with physical frailty, chronic health issues, and/or cognitive impairments, including a Senior Day Program, WOW Waterloo (telephone based group programming), a suite of Home Support Services (transportation, homemaking, home maintenance, telephone reassurance, community dining, shopping) and LEGHO (Let's Go Home), a hospital to home discharge support program.
  • Lead the recruitment, selection, orientation, and development of the various staff teams; Supervise daytoday performance and provide ongoing feedback and coaching; Provide oversight and performance support of students and volunteers as necessary
  • Oversee program operations within legislation, regulations and approved policies and procedures. This includes knowledge of and compliance with accessibility and safety legislation (AODA, OHSA), Privacy legislation (PHIPA, MFIPPA), Connecting Care Act, Convenient Care at Home Act, Patient Bill of Rights, Safe Food Handling (Public Health regulations), as well as all relevant safe work practices.
  • Assist in the development of annual budget and Capital budget for Ontario Health Funded programs and work in conjunction with Finance for the submission of all financial documents and reports required; Liaise directly with Ontario Health to ensure all documentation is up to date and approved
  • Assist to establish, monitor and achieve goals and objectives as set out in the corporate Strategic Plan and Community Programming and Outreach Services Business Plan and Multi-Year Accessibility Plan; Development of section work plans to align with departmental and strategic plans.
  • Provide additional support and resources for clients and families; Collaborates, coordinates and liaises with various community agencies and other health care professionals as required and appropriate.
  • Support the administration of program waitlist, intake and discharge functions in collaboration with staff.
  • Work alongside fleet to ensure the safe operation of two fleet vehicles, including completion of routine maintenance and inspection. Ensure drivers are practicing and following transportation procedures.
  • Participate regularly as a member of various community network groups and partner to ensure smooth client transition within the health system.
  • Provide support and direction during client related incidents, provide guidance to staff teams involving expressed concerns with changes in a client's health or wellbeing.
