Structural Heart Program
at our Bayview Campus**.

Hours of Work:
Weekdays
:Days, 8hrs; As per schedule

Summary of Duties
As an active member of the Structural Heart Program, the Waitlist Management and Structural Heart Coordinator will report to the Patient Care Manager of the Cardiac Cath Lab and is responsible for facilitating timely access to structural heart procedures and managing the structural heart procedures waitlist

Including:

  • Triage and booking of all referrals related to the structural heart program
  • Liaison with patients and referring physicians regarding procedures
  • Collaborate with other Cardiac Care Coordinators, Physician Leads, Nurse Practitioners, Medical Director and affiliated team members for the development, implementation and evaluation of integrated services in the structural heart program
  • Waitlist management: Including development, implementation, and ongoing maintenance of a current, complete, and accurate database for patients awaiting procedures. This includes tracking patient wait time and stability while awaiting their procedure, maintaining current waiting lists, and identifying patients exceeding the recommended maximum waiting time.
  • Collaborates with physicians and teams to notify patient status changes and coordinate patient procedures according to patient needs and alignment with recommended wait times.
  • Patient surveillance, history and physical assessment
  • Data collection and analysis
  • Quality improvement projects
  • Patient and family support and education
  • Participates as a member of the Cardiac Care team
  • Participates in CorHealth activities
  • Bed flow management within Cardiac Cath Lab and Outpatient areas
  • Provides recommendations toward the establishment of sustainable systems and processes, as required, to assist referring physicians within the hospital and between hospitals to assist in timely patient access to services.
  • Identifies and participates in addressing system gaps impacting the quality of patient care. This may include preadmission patient education, communication regarding waitlist status, ongoing telephone support to waitlisted patients, coordination on inpatient versus outpatient scheduling priorities, coordinating triage rounds to ensure the most urgent cases take priority and ensuring improvement to waitlist management.

Qualifications/Skills

  • Baccalaureate Degree in Nursing required
  • Current Ontario College of Nurses Certificate of competence
  • Current BCLS
  • Minimum 5 years recent experience in cardiac required (within last two years)
  • Previous coordinator/TL experience required
  • Statistical data management and quality assurance
  • Ability to integrate the concept of patientfocused care into clinical practice
  • Computer experience essential
  • Demonstrated excellent interpersonal, written and verbal communication skills and ability to work in a team environment
  • Excellent clinical assessment skills
