Job Description:

Reporting directly to the Director, Outpatient Patient Medicine & Regional CKD Program, the Clinical Educator acts as an integral member of the Program's leadership team providing educational support, coaching and mentorship to members within the outpatient medicine clinics, endoscopy and diabetes programs across all sites including Credit Valley Hospital, Mississauga Hospital and Queensway Health Centre.

Key Qualifications:

Current registration with the College of Nurse of Ontario and in good standing Bachelor of Science in Nursing is required with a minimum current enrollment in a Master's degree program or plan to enrol within the year, or combination of equivalent work experience. Minimum five years clinical experience 5 years' experience working with complex medical issues and chronic disease management is required Experience working in Endoscopy and/or Diabetes outpatient programs is preferred. Previous experience in developing and delivering educational programs Computer skills, . PowerPoint, Excel, and Prezi Excellent communication, presentation, and facilitation skills – both written and verbal Understanding of adult education and change management techniques and ability to prepare and teach in both classroom and clinical settings Ability to coach, mentor and provide feedback while fostering a positive work culture Strong critical thinking, decision making, problem solving and conflict resolution skills Evidence of ongoing professional development Commitment to evidence-based practice and quality improvement Good project management and processes/skills

Responsibilities:

Promotes the design, implementation and evaluation of orientation and other educational programs to support excellence in clinical practice and professional development of staff within the Ambulatory Outpatient Clinics, Endoscopy and Diabetes Leads and supports quality care initiatives/ improvement projects through collaboration with members of the Interprofessional team, and uses implementation and evaluation processes to foster best practice and enhance knowledge, skills and professionalism of clinical staff Supports the Patient Care Manager in the selection and appraisal of nursing, allied health and clerical staff as requested Provide mentorship and support development of clinical leaders, charge nurses and allied staff as required Supports and fosters the mission, vision, and values of Trillium Health Partners, Cancer Care Ontario and the Diabetes Association through education, clinical practice, and research processes
