Position Summary:
Washagamis Bay First Nation is currently seeking a full-time Jordan's Principle Service Coordinator. The Jordan's Principle Service Coordinator is to provide support and services to assist First Nation children and families with case management, intake, assessment, application processes, and coordination to navigate the full range of federal, provincial, and local health, social, and education services and supports to address children's needs. The Jordan's Principle Service Coordinator will be the overall manager of the community social and health programs and will insure the delivery of activities services outlined in the 3 Year Work-Plan. They will also manage all community-based individual Jordan's Principle applications. Finally, they will coordinate the delivery of the program capacity development plans.

Duties of this LEAP Job Posting:
- Familiarize self with Jordan's Principle, service coordination and planning service workflow processes.
- Coordinate the development and implementation of the First Nation's Jordan's Principle Social and Health Group proposal.
- Conduct a needs assessment to understand the specific needs of the individuals.
- Collaborate with relevant agencies to develop and implement service delivery plans in alignment with the principles and objectives of Jordan's Principle.
- Coordinate and facilitate the provision of services and supports to Indigenous children, ensuring their unique needs are being met.
- Identify the necessary resources, such as funding, personnel, equipment, and materials, required to implement the service plan effectively.
- Monitor the effectiveness of services with families.
- Ensure adherence to timelines and deadlines for program implementation.
- Coordinate/engage with service providers to share knowledge and processes of coordination and funding.
- Provide community engagement meetings and workshops about Jordan's Principle.
- Provide detailed information about eligibility criteria, application processes, available services, and the rights of First Nations Children under Jordan's Principle.
- Distribute printed materials, such as pamphlets, brochures, and fact sheets, that outline the key aspects of Jordan's Principle.
- Manage the individual cases to ensure that families needs are assessed, documented, and addressed appropriately.
- Maintain accurate and up-to-date records of their interactions with families, service providers, and other stakeholders.
- Responsible for the preparing reports, documenting service gaps or challenges, and providing regular updates to supervisors or funding agencies.
- Collaborate with a wide range of professionals and organizations involved in the care of First Nations children, such as healthcare providers, educators, social workers, and community.
- Attend any required relevant pre-employment training and ongoing training as requested by Chief and Council.
- Attend staff meetings as required.
- Other relevant job duties as assigned by Chief and Council
In order to be considered for this position, applicants must possess any of the following:
- Possess a Grade 12 diploma or equivalent.
- Post-secondary certificate, diploma, and/or degree in the Health Services or Education field.
- Demonstrated knowledge of, or experience related to, special needs of children and youth.
- Ability to prioritize and plan work activities and use time efficiently.
- Proficiency with computers and software applications, such as Microsoft Office.
- Strong interpersonal and communication skills, both verbal and written.
- Experience managing programs and staff
- Must be able and willing to maintain confidentiality.
- Must be reliable and punctual.
- Ability to work flexible hours.
- Possess a valid G Class driver's license and have access to a reliable vehicle.
- Ability to cooperate with other staff members, parents, and community residents.
- Demonstrates diligence, resourcefulness, initiative.
- Must be able to provide a clean Criminal Record Check and Vulnerable Sector Check.
