Victoria, Canada Family Caregivers of BC Full time

It Takes a Village…

Are you passionate about helping people find the support and resources they need? Are you ready to join a dynamic, team-driven organization that truly cares about the communities it serves? If you’re based in Greater Victoria and want to make a difference in the lives of family caregivers, this is your chance to be part of a groundbreaking initiative transforming health and social care. The Caregiver Support Specialist – Community Navigator is a brand-new position, helping family and friend caregivers (people that provide unpaid care to a family member, friend or neighbour) access resources and create wellness plans that address their emotional, social, and practical needs. PLEASE NOTE: This is not a home support worker position.

About Family Caregivers of BC

Family Caregivers of British Columbia (FCBC) is a charitable organization dedicated 100% to supporting family and friend caregivers. We believe it takes a village to provide care, and it is our mission to improve the quality of life for family caregivers through support, information, and education. We provide leadership to strengthen the voice of family caregivers and the significance of their role. We also believe deeply that our ability to fully serve all family caregivers rests on having a welcoming, inclusive, and diverse team who recognizes and appreciates individual differences. We build on each other’s strengths and talents to achieve our goals.

About the Role

The Community Navigator is a key role in the Caregiver Rx Social Prescribing Project, focused on supporting family caregivers by addressing their whole-person needs in collaboration with the health and social care sectors. Central to the role is the delivery of individualized support (by phone, virtual and in person), helping caregivers manage immediate practical concerns as well as ongoing needs that contribute to a caregiver’s physical, social, and emotional wellbeing. This includes providing emotional support, assistance with navigating health and social services, and connecting caregivers with community resources tailored to their needs, such as mental health, financial support, housing, social opportunities, exercise, and nutrition. The Caregiver Navigator will build meaningful ongoing relationships with caregivers, co-create wellness plans, and use motivational interviewing and action planning to empower caregivers in reaching their wellness goals. Reporting to and working closely with the Caregiver Rx Social Prescribing Project Lead, the Caregiver Navigator will also play an integral role in developing and maintaining referral pathways into our program within the wider health and social care community.

Key Responsibilities:

  • Contact caregivers in response to incoming referrals: provide empathetic 1-1 emotional support, assistance with navigating the health care system, formal intake assessment, and/or referral to community resources.
  • Use screening tools to assess caregivers’ strengths, emotional capacity and stress levels.
  • Co-create wellness plans to address a caregiver’s whole-person needs.
  • Assist with problem solving and creating action plans where appropriate, using a Brief Action Planning approach.
  • Assist with accessing services/resources where appropriate.
  • Liaise with community programs and maintain outgoing referral and feedback pathways.
  • In collaboration with team members, regularly update community resources.
  • Share insights on caregiver needs to inform the development of tools and resources.
  • Maintain client record data in a timely and accurate manner via the FCBC database (Salesforce-NonProfit based).

Qualifications and Experience:

  • Degree in healthcare field, social work, social services, geriatric care, mental health or other related discipline, or a combination of education and experience
  • 2 years case management experience (or similar) supporting individuals and/or family caregivers.
  • Excellent interpersonal and communication skills (verbal and written), with a demonstrated ability to provide empathetic, warm, and professional support that is accepting and inclusive of all clients.
  • Experience applying motivational interviewing techniques to support others is an asset.
  • Experience working with groups an asset, with facilitation skills, preferably in a workshop environment. Presentation skills an asset.
  • Caregiving experience an asset.
  • Proficient in Microsoft Office: Word, Excel, Outlook and must have a high level of comfort using online web platforms and digital tools (and the ability to simplify processes for technologically unskilled users).
  • Ability to maintain accurate and timely client records; familiarity with databases such as Salesforce (or similar CRM systems) an asset.
  • Demonstrated ability to work independently and as part of a team.

Location of Work: Victoria, BC at FCBC’s office, as well as hybrid work at home and in the community. Hours of Work: Typically working during our office hours Monday – Friday, 8:30 AM – 4:00 PM and occasionally after-hours for caregiver appointments or other FCBC activities.

Compensation: Commensurate with experience in the range of $27 - $33 per hour. Position type: Full-time, permanent.
