Halton@Home Integrated Care Coordinator

The Halton@Home Program provides enhanced levels of home care services to patients to return safely to their home on Halton@Home program (or patients at risk for ALC) in hospital. The HHICC will work collaboratively with Halton Healthcare’s interdisciplinary team to identify patients appropriate for the program (multiple pathways), coordinate home care services, and ensure a seamless transition from hospital to home with the hospitals and Connected Care Halton’s approved Home Care Service Provider Organization (SPO). In addition, they will provide oversite and care coordination/navigation in the community for of all patients enrolled in the program until they are discharged from the program.
Work collaboratively with Halton Healthcare’s interdisciplinary team to identify patients appropriate for the program (multiple pathways), coordinate home care services, and ensure a seamless transition from hospital to home. Key responsibilities: Care plan development, coordination, and monitoring of patients (Community Caseload), establishing workload priorities, arranging team and family conferences, participating on various committees.
Qualifications
An undergraduate degree in a health-related profession required Regulated Health Professional with a current membership in good standing with a regulatory health professional body in Ontario required 2-5 years work experience in a hospital with liaison with community health setting 2-5 years recent work experience in a hospital setting Demonstrated ability to multitask in a fast pace environment Demonstrated ability to work independently and collaboratively with an interdisciplinary team, patient & family members, support networks, community service providers and a variety of formal and informal service providers. Demonstrated ability and experience to work with diverse patient groups (, multicultural, mental health, geriatrics, etc.) Excellent leadership, interpersonal, communication, organizational and decision-making skills Comprehensive knowledge of community resources and health care delivery systems Meditech (EMR) experience preferred CHRIS (EMR) experience preferred
