Reporting to the Manager ‑ Emergency, the Patient Flow Co‑ordinatorwill be responsible for maximizing utilization of

resources by participating in defining clinical outcomes andestablishing a plan for required services and care. As

part of a multidisciplinary health care team, you will practicewithin the Standards of the College of Nurses of Ontario,

the Philosophy and Values of Nursing Services and the policies ofthe Hospital.

Applicants must have the following qualifications:

* Current registration as a Registered Nurse with the College ofNurses of Ontario

* Current certification in BCLS

* Five years acute care

* Training & demonstrated experience in Interqual ConcurrentReview

* Demonstrated experience in Discharge Planning Coordination

* Knowledge of external agencies and facilities

* Ability to coordinate activities of health care team regardingdischarge planning and Utilization

* Proficiency in groupwise, excel and Meditech software

In addition, applicants must have skill and experience in thefollowing elements of nursing practice:

* Strong analytical skills

* Excellent communication skills

* Patient and family teaching

* Critical thinking, problem solving and conflict resolution

* Clinical assessment skills re: geriatrics, palliative care& rehabilitation

Salary Range: $43.070 to 60.000
