About this opportunity

Reporting to the Director, Clinical Service Design, Home Health Services, the Senior Manager, Transitional Programs is accountable for the high performance and excellence of all Home Health Ontario Transitional Programs. Through developing and fostering strong internal and external relationships, the Senior Manager of Transitional Programs will be vital in the sustainability and growth of existing programs as well as the implementation, evaluation and oversight of new programs. This role will work closely with the business development and design teams as well as internal stakeholders to ensure that CBI Home Health delivers the best possible experience and value for money via our programs that support patient transitions.

Take an inside look at what you’ll do each day

Act as the primary point of contact for local operational leaders, funders and delivery partners
Maintain a solid understanding of external stakeholder needs to ensure CBI Home Health is well positioned to respond to opportunities with innovative transitional program solutions.
Ensure compliance with contractual/partnership agreements
Identify opportunities for program improvement and engage stakeholders to implement changes
Identify partnerships to enhance existing programs and support effective and sustainable transitions
Ensure the collaborative development and effective implementation of pre-discharge and transition planning activities with partners
Ensure transitional lead roles are filled with attention to fit based on Key Accountabilities and Success Profiles, utilizing the appropriate hiring process
Lead by example to coach, mentor and support the growth and development of staff,
Create an environment that draws and retains high performing talent that live the values of CBI Home Health
Ensure career paths and leadership development are attainable

What you need to be successful

Regulated Health Professional registered and in good standing with the respective College, (Occupational Therapist, Physiotherapist or Registered Nurse preferred)
Postgraduate education such as an MBA or advanced university/executive training considered an asset
7 years experience in a general management role in a service-focused organization, senior management level preferred
Ability to lead teams and individuals to achieve results
Demonstrated success with program evaluation and improvement
Experience with clinical program monitoring and financial management
Possess excellent communication and organizational skills with the ability to work as part of a team in a fast-paced environment
Have strong knowledge of quality improvement strategies, health care system, and experience in home and community care
Be comfortable interacting with senior level stakeholders within partner organizations
Combine a demonstrated patient-centred focus with a commitment to providing quality care
