Rexdale Community Health Centre (RCHC) is a non-profit, multi-service, community-based organization that serves communities in Rexdale and North-West Etobicoke. Operating out of three locations, RCHC offers core services that include primary health care, health promotion, community development programs, Pathways to Education and Diabetes Education Centre.
Position Summary:
The Decision Support & Quality Improvement Lead provides strategic leadership and operational expertise in performance measurement, evaluation, accreditation, and research. This role ensures that organizational data is collected, analyzed, and used effectively to drive continuous quality improvement, inform evidence-based decision-making, support accreditation readiness, and advance research initiatives that enhance health outcomes and equity. This role reports to the Executive Director.
Key Responsibilities
Decision Support & Data Analysis
- Lead the development and maintenance of dashboards, scorecards, and performance reports for senior leadership, Board, and funders.
- Conduct statistical and qualitative analysis of clinical, community, and organizational data to identify trends, gaps, and opportunities.
- Ensure data quality, integrity, and compliance with privacy legislation and reporting standards.
- Provide interpretation of data to guide planning, resource allocation, and program improvements.
Quality Improvement (QI)
- Develop and implement organization-wide QI plans, aligned with strategic priorities and funder requirements.
- Facilitate QI projects across clinical, health promotion, and community programs using tools such as PDSA cycles, LEAN, and logic models.
- Build staff capacity in QI methodologies and foster a culture of continuous improvement.
- Monitor and evaluate outcomes, ensuring learning is translated into practice and sustained.
Accreditation
- Lead the preparation, coordination, and follow-up for accreditation processes.
- Maintain accreditation standards and evidence requirements across programs and services.
- Provide training and guidance to teams to ensure compliance with accreditation frameworks.
- Monitor action plans post-accreditation to embed improvements.
Research & Evaluation
- Lead and support applied research projects in partnership with academic institutions and community partners.
- Develop evaluation frameworks and tools for organizational initiatives.
- Translate research findings into actionable recommendations to improve care delivery, health equity, and population health outcomes.
- Promote knowledge translation and dissemination through reports, presentations, and publications.
Qualifications
- Master's degree in Health Administration, Public Health, Epidemiology, Health Informatics, or a related field, or equivalent education and experience
- 3-5 years of progressive experience in decision support, quality improvement, evaluation, or research in a healthcare/community setting.
- Strong knowledge of QI frameworks (LEAN, Six Sigma, PDSA, etc.) and accreditation standards.
- Expertise in data management and analysis tools
- Excellent communication and facilitation skills, with the ability to translate complex data into clear insights.
- Demonstrated ability to manage multiple projects, lead cross-functional teams, and engage diverse stakeholders.
Salary: $100,039 - $120,064 based on experience, plus HOOPP and Benefits
Hours: 35 hours per week
