JOB SUMMARY

The Outreach Community Support Worker (OCSW) operates primarily in a street-based setting in Quinte West, ON, engaging with marginalized individuals to offer support and connect them with services. Although field-based, the OCSW is part of the BQWCHC Community Health Team and has access to office space at their Trenton location.

KEY AREAS OF RESPONSIBILITY

  • Engage with street-involved, precariously housed and marginalized individuals living in Quinte West who are experiencing barriers to accessing care;

  • Work to establish trusting ongoing relationships and connections with individuals at risk of poor health outcomes and precarious/at risk social circumstances;

  • Assist clients to identify a prioritized action plan for addressing needs and to help reduce situational crises and ongoing instability;

  • Facilitate awareness of and access to existing resources, groups, agencies and other service providers in support of clients and collaborate with such resources in delivery of such services to clients;

  • Work in a manner that incorporates health promotion and recognizes the social determinants of health; and

  • Other duties as appropriate/assigned

QUALIFICATIONS

  • Prior experience with individuals who may be street-involved, homeless, precariously housed, struggling with mental health, problematic substance use a definite asset, preferably in a street-based model

  • Minimum 3 years' relevant experience in a community setting, or combination of community, public health or social services experience

  • Familiarity with health and social services in community of Quinte West

  • Philosophy and values consistent with person-centered and harm reduction approaches to care

  • Excellent time management, problem solving and networking skills

  • Experience with electronic documentation and data collection

  • System navigation skills

  • A valid driver's license; reliable vehicle and willingness to travel within the Quinte area is required

  • Ability and willingness to work independently at times in an unstructured street-based model, while maintaining vigilance for the importance of the strategic goals of the program
