Summary

The Home Help Service Coordinator will be responsible for arranging in-home services and assistance to seniors and persons with disabilities. The Service Coordinator will help plan the type and length of support needed in consultation with the client(s). He or she will be tasked with conducting initial assessment, service planning and conducting follow up meetings to verify that the needs and requirements of individuals and their families are being met. The work will vary depending on the particular needs of the service recipient and may involve liaising with other Family Service Kent departments and/or community partners. Additional duties will include creating and maintaining case files in a confidential manner. Other duties may be assigned as necessary.

Job Duties

  • Conducts assessment of the needs and requirements of clients and their families.
  • Provide service coordination to seniors and persons with disabilities to assist them in relevant area(s).
  • Travel as required to clients home for preliminary assessment of needs.
  • Meet with clients and/or family members to establish goals and create service plans which include a clear understanding of service delivery expectations.
  • Assist client and families with connection to appropriate service providers, support services in the community.
  • Cooperate with other agencies and professionals to coordinate services for mutual clients. Work collaboratively with interdisciplinary teams.
  • Conduct regular follow up calls; make field visits with client and their families.
  • Screen Independent Resource Contractors ("Home Helpers"), complete individual interviews, complete reference checks and verify criminal records check
  • Broker Independent Resource Contractors ("Home Helpers") with Home Help clients & provide liaison services.
  • Participate in agency meetings, agency and relevant team activities, and in-service training as required for the position. On-going training and career development is encouraged.
  • Recognize potential client crisis; analyze circumstances and dynamics with sound judgment.
  • Develop strategies to deal with risk, crises and unusual incidents.
  • Create and maintain client records in a confidential and professional manner according to Family Service Kent policies. Record case notes according to accepted practices and policies. Document incidents and inform all individuals and agencies as necessary
  • Compile program statistics.
  • Practice quality of service as well as health and safety best practices.
  • Use accreditation documentation practices; attend accreditation program delivery training and reviews.
  • Meet regularly with Program Manager to assess progress, program effectiveness and debrief; keep Program Manager informed of outcomes.

Performance Factors

The following factors, among others, will be used by the Community Support Services Manager in evaluating the performance of the incumbent annually:

  • The degree to which CSS services are delivered in accordance with agency philosophy, policies and procedures and funder guidelines;
  • Staff morale, satisfaction and commitment;
  • The quality and effectiveness of service delivery.

Requirements

  • Post-secondary training; preferred degree in health care, human services, social work or a related field and/or an equivalent combination of post-secondary education and experience is required.
  • A minimum of two years working with seniors and/or persons with disabilities.
  • Combined knowledge and experience about community services that support and assist seniors and persons with disabilities.
  • Working knowledge of brokerage models, case management, and risk management.
  • Capable of understanding written manuals and directives.
  • Strong knowledge of applicable home care legislation, regulations and guidelines.
  • Exceptional conflict resolution.
  • Highly flexible, with solid interpersonal skills.
  • Excellent problem solving skills.
  • Strong work ethic and positive attitude.
  • Excellent teamwork and team building skills.
  • High level of attention to confidentiality.

  • Valid Criminal Records Check
  • Must possess a reliable vehicle for work and appropriate insurance coverage

Work Conditions

  • Interacts with clients, family members, staff, visitors, government agencies/personnel under all circumstances, which may be traumatic situations.
  • Manual dexterity required to use desktop computer and peripherals.
  • Intermittent physical activity including walking, standing, sitting, lifting and supporting clients.

Job Type: Full-time
