Client Intervention Coordinator
Temporary, Full-Time (1 year) - 30 hours/week
Overview
The Client Intervention Coordinator provides short-term, person-centered support services in the community to
vulnerable and at-risk seniors and adults living with disabilities. The Client Intervention Coordinator seeks to empower
individuals, and help those individuals resolve problems or address issues which may be compromising their social
functioning and/or impacting negatively on their quality of life. The scope of service is broad, and might involve
well-being appear to be at risk.
Job Duties
Gather background information through assessments and interviews with service users.
Receive and process community-based and self-referrals to the program.
Assess client needs and develop an appropriate action plan for assistance based on apparent need and/or
client goals.
Connect service users with appropriate community partners, such as housing, transportation, medical or legal
assistance.
Assist clients with applications for benefits and social service programs.
Advocate alongside clients for adequate, timely and cost-effective services.
Meet with clients periodically to assess their progress, discuss any difficulties they are facing, and develop
solutions.
Create and update client records and files, prepare notes, case documents and reports.
Maintain involvement until client-identified goals are reached, and/or a suitable follow-up plans are in place.
Contribute to community awareness regarding important issues relevant to this program area (e.g. elder
abuse, hoarding, etc.).
Collect program statistics related to funding criteria.
Work collaboratively with other service providers to meet client-identified goals related to social
determinants of health.
Represent Family Service Kent on appropriate community committees as required (e.g. BSO, EAPO)
Collaborate with the other members of the CSS team, to participate in the development of program goals and
the program service plan, and to ensure the accomplishment of program goals.
Requirements
BSW or equivalent training in a related discipline.
1-2 years relevant experience
Demonstrated ability to respectfully assist clients, family members, and the community with care and concern,
consistent with the Core Values, Vision, and Mission.
