Intake Triage and Assessment Coordinator

The Intake, Triage & Assessment Coordinator will review, assess and respond to all incoming referrals by telephone, walk-in and email. The Coordinator will provide services or supported referrals to Social Workers in the program or partners in the program. The Coordinator will also provide information, and assist seniors with accessing services and system navigation. The emphasis of the program is providing social supports to assist the older adult to remain healthy in a holistic way in the community.

QUALIFICATIONS, SKILLS & EXPERIENCE REQUIRED
- Degree or Diploma in a Human Services field
- Knowledge of issues impacting seniors and a working knowledge of community resources in Edmonton for older adults
- Excellent organizational skills; strong attention to detail
- Self-motivated; able to work independently and as part of a team
- Computer literacy including Microsoft Office
- Proficient communication, assessment and collaboration skills
- Police Information Check including Vulnerable Sector is required
- Experience working with older adults, volunteers and diverse cultures
This position is a permanent, full-time in-person (35 hours/week) position. The salary range for this position is $42,448 - $50,298 plus 100% Employer paid Health & Dental plan, and 5% matched Pension plan. Salary considerations are based on an assessment of the successful candidate's education, skills, previous experience and other factors. Sage Seniors Association offers an outstanding work life balance with 3 weeks of vacation to start, a 35-hour workweek, generous sick leave accruals, and paid time off between Christmas and New Year's Day. Sage also observes 13 statutory holidays in the calendar year.Sage is offering significant flexibility through a hybrid working environment (remote Fridays) and the option for a compressed work week. Social Workers are re-imbursed for their annual ACSW registration fee as well as Social Worker Liability Insurance premiums.
