The Opportunity:

We are recruiting a MENTAL HEALTH CLINICIAN (FACT)

  - This is a regular full-time opportunity (35 hours per week Monday to Friday 9am to 5pm) based at 80 Waterloo Ave. Guelph.

As a member of a flexible, interdisciplinary treatment team, you will provide recovery oriented counseling and support to individuals with severe and persistent mental health challenges and their families.  This will include providing support, comprehensive assessments, clinical services, consultation and collaboration with community partners and group support.  In keeping with the FACT standards, a comfort in providing assertive outreach, engagement, home visits and community-based care is a must.  This position reports to the Manager, Adult Intensive Services. 

Flexible Assertive Community Treatment (FACT) is a recovery-oriented, evidence-based best practice. A  FACT team is comprised of a multidisciplinary team of clinicians monitoring and treating clients with severe and persistent mental illness (SPMI).  The target client population is those who require both assertive community treatment (ACT) with a larger majority of clients on the team who need less intensive treatment and support and could be served in a traditional intensive case management model. To combine care for these two levels of care, the FACT team employs a flexible switching system where the intensity of services can be ramped up or ramped down as needed depending on individual client need. This combination of flexibility and continuity provides better opportunities for recovery and rehabilitation and ties in well with the natural course of SPMI with its recurring episodes and relapses.

Qualifications:

  • Masters level degree in psychology, social work or relevant health or social science field
  • Registration with relevant college/registering body
  • 3 to 5 years' experience within the health services sector with in-depth knowledge of complex mental health and addictions issues
  • Knowledge and experience in providing harm reduction techniques for clients with addictions
  • Sound understanding and demonstrated knowledge of the needs of people with mental health concerns and their families, and the programs and services in the community
  • Knowledge and demonstrated skills in working with individuals who have substance use issues, including motivational interviewing
  • Experience and understanding of how concurrent disorders and social determinants of health impact the lives of individuals
  • Demonstrated experience and skill in providing both individual and group therapy
  • Knowledge of various modalities of therapy including DBT, Trauma Informed, MI
  • Demonstrated understanding and sensitivity to social and cultural diversity
  • Highly developed organizational, communication and problem solving skills
  • The ability to work collaboratively in a self-directed work team
  • Proficiency with MS Office, Outlook and various database software
  • Valid driver's license and own transportation as travel is a component of the position
