As a member of the HIV and Gender-Affirming Care multidisciplinary team, the Mental Health Clinician is responsible for providing counselling, case management, family and group work services within a holistic, health promotion framework. The MH Clinician sees clients through both scheduled and urgent appointments, through Guelph CHC, outreach and home visits in the community, during regular and extended hours. The Social Worker will work in close partnership with local AIDS Service Organizations to support integrated, wrap-around, team-based care support for clients.
**Main Responsibilities**
- Supports the functioning and independence of clients by providing psychosocial counselling services to individuals, families and groups.
- Supports clients to access community support, and health, social and recreational services, by acting as a client advocate.
- Acts as a resource person to Guelph CHC staff, clients and community residents by compiling information about community resources and supports access and connections to those resources by liaising with community groups.
- Works with clients to develop comprehensive treatment plans by providing information to other Guelph CHC professionals on an ongoing basis, by participating in case conferencing, and by providing consultation on psychosocial issues to Guelph CHC staff.
- Supports the implementation of appropriate treatment, and maintains client records, assessment notes and correspondence within the safeguard of confidentiality.
- Contributes to the development of new programs that address community health needs. Collaborates with Guelph CHC staff and other agencies to identify, recommend and/or develop resources and materials, and participates in delivery of the program.
- Ensures that services are evaluated.
- Provides leadership for appropriate community educational programs
- Supervises students in field placement practicums.
- Participates in community groups and committees relating to the development and delivery of health care and social services in the community.
- Responds to codes (specifically white or yellow) and helps to manage social/emotional or mental health crises that present in clinic, as well as provides walk-in/on-call counselling services.
**Key Qualifications and Skills**
- Master of Social Work or relevant post-graduate degree.
- Registered regulated College in Ontario which allows provision of psychotherapy.
- Two to five years’ experience in a community based human service organization.
- Thorough knowledge of, and proficiency in, current assessment, psychosocial counseling techniques and group work skills.
- Thorough knowledge and understanding of community resources, preferably Guelph and Wellington County resources.
- Demonstrated expertise in client-centred, trauma-informed care and understanding of the value of interdisciplinary care in a health care setting.
- Proven cultural competence and commitment to health equity, with ability to work effectively with diverse populations.
- Ability to set goals and get results within a set timeline.
- Ability to multitask and work independently.
- Effective written and oral communication skills.
- Effective time management, decision making and organizational skills
- A good team member and supportive of interdisciplinary practice.
- Experience working with people living with HIV/AIDS, people who identify as Transgender and people seeking gender affirming care, considered an asset.
- Lived or living experience considered an asset.
- High level of confidentiality.
- Knowledge of privacy and confidentiality standards on Ontario, including PHIPA.
