As a member of the multidisciplinary team, the Mental Health Clinician is responsible for providing counseling, case management, family and group work services within a holistic, health promotion framework. The Mental Health Clinician sees Health Centre Clients, with both scheduled and urgent appointments, on-site at GCHC locations and through outreach and home visits in the community, during regular and extended hours.
**Core Responsibilities**:
- Maximize the functioning and independence of clients by providing psychosocial counselling services to individuals, families and groups.
- Ensure clients receive community support, health, social and recreational services to which they are entitled, by acting as a client advocate.
- Ensure comprehensive treatment for clients by providing information to other GCHC professionals on an ongoing basis, by participating in case conferencing, and by providing consultation on psychosocial issues to GCHC staff.
- Ensure appropriate treatment is delivered and maintains client records, assessment notes and correspondence within the safeguard of confidentiality.
- Contribute to the development of new programs that address community health needs.
- Collaborate with Guelph CHC staff and other agencies to identify, recommend and/or develop resources and materials, and participates in delivery of the program.
- Ensure services are evaluated.
- Participate in community groups and committees relating to the development and delivery of health care and social services in the community.
- Respond to codes (specifically white or yellow) and helping to manage social/emotional or mental health crises that present in clinic, as well as providing walk-in/on-call counselling services.
**Required Qualifications**:
- Master of Social Work, or relevant post-graduate degree
- Registered with a regulated College in Ontario which allows provision of psychotherapy.
- Two to five years’ experience in a community-based, human-service organization.
- Thorough knowledge of, and proficiency in, current assessment, psychosocial counseling techniques and group work
- Thorough knowledge and understanding of community resources, preferably Guelph and Wellington County
- Ability to set goals and get results
- Effective written and oral communication skills
- Effective time management, decision making and organizational
- A good team member and supportive of interdisciplinary practices
- Knowledge of privacy and confidentiality standards on Ontario, including PHIPA.
