The Indigenous Care Coordinator , works in partnership with a variety of healthcare providers for chronic disease management, including the Renfrew County Diabetes Education Program and the Renfrew County Mesa HART Hub, and with Indigenous patients and caregivers and the broader Indigenous community to enhance and improve the experience and the health outcomes for people with complex health conditions, including diabetes, prediabetes or other vascular health or chronic disease condition within Renfrew County.

The role of the Indigenous Care Coordinator is to build trust, support and empower the patient by providing navigation and care coordination, through the healthcare system. Responsible for establishing linkages between the indigenous community, traditional health and healing practices and our current health care system, the Indigenous Care Coordinator will promote indigenous services, including the role of the Indigenous Care Coordinator, identify systems gaps, make cross-sector partnerships and advocate on behalf of the patients to assist in meeting their physical, social and cultural needs for healing.

While maintaining an active patient caseload, the Indigenous Care Coordinator will also advocate for the Indigenous community in healthcare, be an active participant in committees, circles or working groups related to Indigenous health or cultural safety and attend and offer education to promote Indigenous Care Coordination and how to establish and maintain culturally safe healthcare practices within Renfrew County.

REQUIREMENTS:

Education:

  • Minimum two (2) year post-secondary education in health, social services or related field
  • Valid Class G Driver's License
  • Diploma or continuing education in case management and/or care coordination is an asset
  • Regulated health professional in good standing an asset
  • Experience and formal training combined with demonstrated performance and ability may substitute for stipulated academic requirements

Experience:

  • Minimum of two (2) years of experience in an acute, primary or community care setting, preference given to candidates with experience in more than one setting,
  • Demonstrated knowledge and understanding of the broader health care delivery system and available resources within the Champlain region,
  • Knowledge of Indigenous communities, history and issues,
  • Experience working with indigenous population or able to demonstrate a strong interest in working with this group, especially related to care coordination and chronic disease management an asset,
  • Experience working in the field of Mental Health and Substance use disorders an asset,
  • Familiarity with patient counselling and interviewing techniques (e.g. Motivational Interviewing) in order to enhance patient care,
  • Demonstrated understanding of the social determinants of health and the impact on patients/caregivers.
  • Experience working in a multidisciplinary team

Skills and Abilities:

  • Has an available, safe vehicle, and clean driver's license record three (3) years prior to date of hire
  • Carries appropriate automobile liability insurance of no less than $2,000,000
  • Ability to work with multidisciplinary care teams that promote cultural safety
  • Ability to work independently with minimal supervision,
  • Excellent communication and interpersonal skills, effective and creative problem-solving skills and commitment to patient-centered care,
  • Ability to prioritize and carry out various job functions ensuring all deadlines are met in a timely fashion,
  • Experience working with and linking to other community health agencies and social service providers, particularly those serving the Indigenous community,
  • Basic knowledge of Chronic Disease Management (with a focus on diabetes and vascular health),
  • Knowledge of Indigenous communities, history and issues
  • Ability to think and act creatively to find ways to assist patients and families/caregivers in meeting their goals ,
  • Proficiency in the use of computers (Microsoft WORD, PowerPoint, Outlook, internet and electronic health records) and willingness to learn advanced computer skills as required,
  • Experience tracking qualitative and quantitative data,
  • Must have demonstrated ability to meet the attendance standards of the Hospital,
  • Embodies the LEAN ideology by actively and continually seeking opportunities for continuous improvement

RESPONSIBILITIES:

  • Working with patients and families to develop a coordinated care plan, prioritizing patient healthcare goals, and incorporating individual cultural and spiritual needs.
  • Hosting and facilitating care conferences with the patient and family/caregiver and multi-disciplinary healthcare providers ensuring that the patient is supported to identify their healthcare goals within the coordinated care plan, linking to internal and external care providers
  • Collaborating with the hospital and community partners to ensure that identified patients receive the services needed to manage their care needs to maintain a healthy lifestyle in their community.
  • Playing a key role in improving patient transitions within the health care system by encouraging collaboration and coordination between members of the patients' care team, especially transitions between hospital and home
  • Advocating on behalf of patients and their caregivers when required to ensure their voice is heard, which may include accompaniment to appointments and home visits as requested/required
  • Working directly with and mentoring care coordination contacts at local healthcare organizations within Renfrew County
  • In collaboration with the Manager of Vascular Health, Mesa HART Hub Team and Spiritual Care Coordinator at Pembroke Regional Hospital, the Indigenous Care Coordinator supports the development and maintenance of effective working relationships with Indigenous Peoples and multiple community partners including Renfrew County & District hospital sites and associated health care teams, indigenous partners throughout Renfrew County, including Algonquins of Pikwakanagan, to ensure an integrated patient-centered regional approach to care services that is appropriate to indigenous patients.
  • Establishing and strengthening collaborative relationships with a variety of health professionals, formal and informal community leaders, and elders.
  • Providing services using a holistic lens to ensure cultural appropriateness while assisting individuals, their families and/or caregivers to better understand and utilize the local healthcare system
  • Actively attending and offering education to promote culturally safe healthcare within Renfrew County
  • Serving on departmental and related committees and/or working groups related to Indigenous peoples, care coordination, Chronic Disease Management and as requested.
  • Participates actively in related quality improvement initiatives.
