The Inpatient Social Worker provides psychosocial assessment, care planning, and treatment coordination for hospitalized patients and their support systems. They address barriers to care, manage complex cases, and collaborate with healthcare teams to support patients across acute care settings.

ROLE

  • Conduct psychosocial assessments and coordinate care and discharge planning
  • Support patients with mental health, addictions, stroke, palliative care, trauma, homelessness, and insurance issues
  • Advocate for access to community resources, financial supports, and housing
  • Provide crisis intervention, brief counseling, and conflict mediation
  • Collaborate with interdisciplinary teams for integrated patient care
  • Assist with advanced care planning and complex system navigation
  • Maintain timely and accurate clinical documentation
  • Make referrals and consult with internal and external professionals
  • Participate in professional development, quality initiatives, and education
  • Ensure patient safety, confidentiality, and infection control compliance

  • Perform other duties as assigned

QUALIFICATIONS

  • Bachelor of Social Work (BSW) from an accredited Institution (Required)
  • Master of Social Work (MSW) (preferred)
  • Registration with the Ontario College of Social Workers and Social Service Workers
  • Minimum 2 years' experience in an interdisciplinary acute hospital setting
  • Experience working with older adults and aging populations, including geriatric assessments and support
  • Experience with mental health and rehabilitation would be an asset

ADDITIONAL INFORMATION

Location: Huntsville District Memorial Hospital (HDMH) located in Huntsville. Applicants must reside within 125km of one of our hospital campuses or be willing to relocate prior to starting. *Exceptions will be considered on a case-by-case basis

Hours of Work: Shifts are typically 8 am - 4 pm. Shifts and hours may change according to departmental requirements per the collective agreement.
