ABOUT THE ROLE
The Social Worker plays a key role in supporting clients, their families, Substitute Decision Makers (SDMs), and other members of their support network. In this role, you will provide comprehensive assessments and interventions to address psychosocial needs, while contributing a social work perspective as part of an interprofessional team. You will also act as a liaison and consultant to internal and external programs within your scope of practice and actively participate in quality improvement initiatives.
The focus of Social Work practice in this program is to restore, maintain, and enhance quality of life by building on client strengths, supporting coping strategies, helping individuals adapt patterns of interaction and daily living, connecting people to necessary resources, addressing environmental stressors, and providing education that promotes wellness and wellbeing.

WHAT YOU BRING
Education/Experience:
- Baccalaureate degree in Social Work (HBSW) from an accredited university with training and/or certification in a mental health preferred
- Current registration with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) and thorough knowledge of its Code of Ethics and Standards of Practice
- Knowledge of social work values and ethics, theory and established practices of clinical social work
- Understanding of the social determinants of health of indigenous people and the social consequences and risk factors associated with client conditions (ie. addictions and mental health)
- Knowledge of community systems and resources for the purposes of networking between the facility/program and other services
Skills/Abilities:
- Ability to manage a caseload in a health care setting
- Ability to complete analysis, using clinical knowledge, skill and judgement to synthesize assessment information
- Assessment skills related to psychosocial issues
- Ability to communicate effectively and concisely, both orally and in writing
- Ability to work independently, interdependently, and participate as a contributing member of a variety of teams within the workplace, community and ministries/agencies
- Strong time management, analytical/critical thinking, problem solving and decision making skills
- Ability to use independent judgment and maintain confidentiality of records, actions, and information
- Demonstrated regular attendance at work
Asset:
- Training and experience related to client care for people from Indigenous, geriatric, addictions and mental health, chronic pain populations would be an asset
Conditions of Employment:
- This position shall be conditional upon a satisfactory Police Vulnerable Sector Check (PVSC) where indicated, to ensure the absence of relevant criminal convictions.
- Offers of employment made to external candidates are conditional upon providing proof of a complete primary series of Health Canada approved mRNA COVID-19 vaccine or proof of valid medical exemption, prior to the offered effective date.
